Service Description

Due to the length of new patient visits, a $50 deposit is required for booking. It is recommended that new patients call the office at (970) 275-5909 upon booking their appointment online to allow us to collect the information necessary to set up your online patient portal. We will send your new patient intake questionnaire via your portal for you to fill out prior to your visit. This will allow the best use of your time with Dr. Robins on the day of your visit.
